Helping seniors stay safe and independent in their own home
Niagara Region offers a range of outreach services to older adults to help them stay in their own homes for as long as possible.
Community workers provide in-home visits to determine what the senior wants and needs to stay at home. Staff can make referrals on the senior's behalf to programs designed to improve safety, independence and quality of life.
Referrals to community services may include:
Individuals living in Niagara, aged 60 or older, or with age-related illness are eligible for these services.
Outreach services provided by Niagara Region Seniors Community Programs are free of charge.
To access these services, call Seniors Community Programs at 905-984-2621 or toll-free at 1-877-212-3922, 8:30 a.m. - 4:30 p.m., Monday - Friday.
A community worker from Seniors Community Programs will call or visit. Visits can be arranged at the senior's home or in an alternate location.
Funding for Outreach Services is provided by the Hamilton Niagara Haldimand Brant Local Health Integration Network.
