Provides community-based treatment, case management and counselling for adults 16 years and older, who are experiencing a serious and persistent mental illness. The program provides a recovery-focused approach and mental health services are provided by a multi-disciplinary team. Services are provided in the home or a community setting of the client's choice. Office visits are also available.
Services
The Clinical Case Management Service
Designed to provide clinical support and counselling to persons with serious mental health needs. Its role includes co-ordinating services between hospital and community, and providing education and support for the client and his/her family.
Individual and family sessions are provided by registered psychiatric nurses, occupational therapists and professional social work staff who assist the client in developing recovery goals and mutually agreed upon treatment plans.
Assistance is also given to clients who are on medication and need information and support around medication use.
Group therapy programs are offered for adults with anxiety and panic disorders, in partnership with the Niagara Alcohol and Drug Assessment Service; and for adults with Borderline Personality Disorder.
- The Intensive Case Management Service
Designed to provide a high level of support to the elderly who suffer from severe and persistent mental health problems. It strives to promote wellness and enhance the quality of life of the elderly mental health client and his/her family.
The team consists of registered nurses, social worker, and community support workers who work in partnership with various organizations presently serving the elderly mental health client. This service provides holistic assessments, the development of care plans that address client goals and needs, education, service co-ordination, intensive support and referral, and community consultation.

The Intensive Geriatric Mental Health Case Management Service
This service is provided in partnership with the Alzheimer Society of Niagara. It is designed to provide a high level of support to the elderly who suffer from severe and chronic mental health problems. It strives to promote wellness and enhance of the quality of life of the elderly mental health consumer and his/her family.
The team consists of registered nurses, family support counsellors and social workers who work in partnership with various community organizations presently serving the elderly mental health consumer.
This service provides holistic assessments, the development of care plans that address consumer goals and needs, education, service co-ordination, intensive support and referral, and community consultation.

The Assertive Community Treatment Team (ACTT)
A multidisciplinary team that provides assertive community-based treatment, rehabilitation and supports for the most seriously mentally ill . The Team works closely with the client, family and other community resources to assist them to gain or regain the valued roles, skills and supports needed, through individualized rehabilitation goals to achieve improved quality of life.

Early Intervention in Psychosis
Provides treatment and multi-disciplinary follow-up to people experiencing early stages of psychosis, in partnership with the Cleghorn Program (St. Joseph's Healthcare). This service is intended to assist clients and their family/friends to identify their concerns and goals, and to develop plans towards recovery from psychosis.
Referral Process
There is a central intake for all services within the Community Mental Health Program. Referrals and inquiries may be made Monday through Friday between 8:30 a.m. and noon, when staff is available. Self-referrals, agency referrals or physician referrals are welcome.
