Children In Need Of Treatment Schedule of Dental Services and Fees

The Children In Need Of Treatment Schedule of Dental Services and Fees (Dentist Providers) is distributed by the Ministry of Health and Long - Term Care and Ministry of Health Promotion to all Ontario dentists, dental clinics and Public Health Departments.

This schedule is used exclusively to complete claim forms.

For a revised or duplicate copy of the Schedule Dental Offices can contact the Ministry of Health and Long - Term Care, Public Health Division at 1-866-532-3161 or 1-416-327-8820.

Or by reviewing the province's Schedule of Services.

The fee schedule lists services available to children identified as having emergency dental conditions who meet the program's criteria as assessed through the Public Health Departments Dental Program.

Effective January 1, 2009, the program will provide dental care for children and teens age 17 and under, up from the previous maximum eligibility age of 14. As well, the program will introduce general anaesthetic coverage for children and teens age 17 and under.

Either a claim form will be given to the parent/guardian to take to the dental appointment or will be mailed to the dental office. The child's personal information must be on the form and is valid only for the specific child for whom it is issued for six months from the date of issue.
